Hello all,

Below is a memo that will be sent out today to all residents and employees regarding our COVID Vaccine Clinic that will be held on Friday January 29^th at 10:00am in the Fair Oaks Auditorium. The second dose will be administered on Friday, February 26^th at 10:00am. I have also attached a copy of the consent form that needs to be completed and signed by each person who wants the vaccine. This form is due to me by Tuesday, January 26^th (refer to memo). Many of the residents are able to complete the consent form on their own. If you feel your loved one is not able to complete the form on their own, please fill out the form and send it back to me by email. I will print it out and have it available for the clinic staff next Friday. Please do not hesitate to get in touch with me should you have any questions or concerns. I am very happy that this process is finally beginning. It brings us one step closer to a “reopen” which will benefit everyone immensely.
